These include personal documents such as a passport, government, state, military, or Tribal ID, birth certificate, Certificate of U.S citizenship or naturalization, U.S Driver’s license, or Green card or permanent resident card.

Eligibility documents are also required, which prove that the applicant either earns a low income or participates in a government-assistance program. These documents may include a current income statement from the employer, paycheck stub, prior year’s federal, state, or Tribal tax return, Social Security statement of benefits, a copy of pension or retirement statement of benefits, or any other document that shows the applicant's income.
